GENERAL REQUIREMENTS:

  1. Two 2x2 colored picture (white background)
  2. Original passport (6 months valid)
  3. Old passport issued within the last 10 years
  4. Original personal bank certificate (must include account type, current balance,
  5. Account opening date, 8 months average daily balance)
  6. Original bank statement or certified true copy of passbook for the last 8 months
  7. Original PSA birth and marriage certificate

     

    FOR EMPLOYEES:

  8. Original employment certificate with leave of absence (including applicant’s position, date hired, salary, date of return to work)
  9.  Pay slips for the last 6 months 
  10. Copy of PRC/IBP ID (if professional)
  11. Copy of company ID
  12. Copy of recent personal income tax return

FOR BUSINESSMEN:

  1. Copy of business registration (DTI or SEC)
  2. Copy of business permit or mayor’s permit
  3. Copy of income tax return
  4. Copy of financial statements

FOR STUDENT MINORS:

  1. Proof of enrollment
  2. Copy of school ID
  3. Leave of absence from school (if traveling while classes are ongoing)
  4. Consent form (signed by both parents, allowing the child to travel)

RETIREE:

  1. Copy of senior citizen card or retirement documents

FOR FOREIGN NATIONALS:

  1. Copy of alien certificate of registration (ACR)
  2. Copy of international card residence (ICR)
  3. Re-entry permit

IF THE COMPANY WILL SHOULDER THE TRIP:

  1. Guarantee letter from company
  2. Original bank certificate and bank statements of the company (latest 6 months)
  3. Copy of business documents and ITR of the company

TRAVEL ITINERARY 

  1.  Cover Letter with Travel Itinerary
  2.  Fligh Reservation Confirmation / Must include  Confirmed Rountrip Ticket 
  3.  Hotel Reservation Confirmation 
  4. Tour Voucher 

ADDITIONAL REQUIREMENTS:

  1. Cover letter explaining the purpose of the trip 
  2. proposed day-to-day itinerary
  • Turkey visa sticker application is applicable for Philippine passport holders who have no valid US visa or Schengen visa.
  • Additional requirements may be required by the embassy without prior notice.
  • Turkey visa processing if applicable for those availing tour package with us.
