NEW Visa requirements – Single Entry
1. Application form
2. Photo
3. Passport
4. Personal Bank Cert – must show the ADB for 6mos
* if ADB is less than 6mos, submit Bank Statement for the last 6 mos.
5. Income Tax Return – must be signed by the applicant
• Form 2316 – with signature of employee and employer
• Form 1700 – with BIR stamp or email from BIR and proof of actual payment
• Form 1701 – with BIR stamp or email from BIR and proof of actual payment
• Form 1702 – with BIR stamp or email from BIR and proof of actual payment
6. Certificate of Employment (must indicate period of employment,
salary and position)
• For business owners – must submit
o Photocopy of Department of Trade and Industry (DTI) “Certificate of Business
Name Registration” / SEC
o Photocopy of Mayor’s Permit from the City Hall (current/valid)
• For Retirees, Housewife, Unemployed
o submit explanation letter for non submission of ITR and Certificate of
employment
• For Students
o submit a copy of the school ID or certificate of enrollment in school
7. Company ID
8. PSA Birth Cert
9. PSA Marriage Cert (if married)
10. Daily Schedule
11. If doctor, lawyer, engr. etc., please
provide a copy of the ID.
